Read and follow all directions on the forms carefully.
Please fill in all forms completely and accurately by TYPING all information.
If not available, please use blue or black ink.
Illegible or incomplete forms will not be accepted.
Once completed, please return the form to the EEE Department Office, RVR 3018.
Please allow at least 72 hours after submission for forms to be processed.
ADVISING & MAJOR DECLARATION

Note: During finals week, the Student Evaluation and Supervisors Report are to be emailed to the Department at firstname.lastname@example.org. (Click Here for Internship Reports)
Failure to turn in these reports will result in an Incomplete grade being issued.
Please Do Not use the EEE 195 forms above if you are enrolled in EEE 195A - Co-Op
FOB KEY FORMS
For all FOB Key Requests - change name, ID, and date of graduation.
FACILITIES SERVICES KEY ISSUE FORM
Graduation Petitions are due one year in advance, and should be submitted online no less than 1 week prior to the University deadline.
The approved substitutions beginning Fall 2013 are as follows and must be noted on the application:
ENGR 1 - you may use ENGR 70, a EEE Lab course, or a EEE Elective course. (EEE 194 or EEE 199 may not be used).
Please note, courses may not be used twice for credit.

If you require a letter from the EEE Department, please choose from the list below. Templates and examples have been provided for you. Please type your letter and email it to the EEE Graduate Coordinator, Dr. Preetham Kumar (email@example.com) AND EEE Dept. Student Assistant (firstname.lastname@example.org). During Summer and Winter break, please email Dr. Kumar ONLY. EEE Student Assistants do not work once the Semester has ended and emails will not be returned during that time from the eeeassistant email.
Once your letter has been received and checked, it will be printed on EEE Department Letterhead and signed by the EEE Graduate Coordinator. Incomplete or letters not completed properly will not be processed and returned to you for corrections. Letters could take at least 3 days after submission to be processed, especially during busy periods. Please be patient. You will be notified when your letter is ready for pick up.
