Read and follow all directions on the forms carefully. Always write legibly and use black or blue ink.
Most of the forms listed below are also available in the CpE Program office, RVR 3018.
Add/
Drop Permit: Use this
form when you can not register through "My Sac State," and until the
University census date only. Remember to attach transcripts if pre-requisites are an issue. (click

Add/Drop Permit After Deadline: Use
this form to drop courses only after the university census date.
NOTE: you will not be allowed to add courses after the census
date. (click
here for census date)
Supervisory Course Petition: Use
this form to add supervised courses - CpE 199, 299 and 500.
Internship Packet
: Use
these forms to add CpE 195, or CpE 295 - Fieldwork in CpE courses.
NOTE: You must have submitted your report and your supervisor's
evaluation for any previous internship period before being allowed
to register again for an internship.
Advising Form: Please fill in form completely before
seeing your CpE Academic Advisor.
Change of Major Form: Use this form if you are changing
you major to CpE, or if you are switching from to Pre-CpE to
CpE. To change from Pre-CpE to CpE, all lower division course
requirements must be completed with C- or better.
CpE WPE Waiver: If
you have not taken the WPE, you will need this form and an add
permit to register for courses that require
the WPE as a prerequisite. NOTE: The WPE is a
University requirement and must be completed to graduate. This
form is only intended to assist you in registering for classes.
Completed forms with appropriate signatures will need to be
turned in to Admissions and Records to be added.
Substitution or Waiver Petition: Use this form to establish
approval of non-articulated course substitutions or waivers.
NOTE: only use this form if you have not submitted a graduation
petition.
Repeat Grade Replacement Petition: For
a single course repeat, use this form. Please read form carefully,
and follow directions on form before turning it in to Admissions
and Records.
Petition for Incomplete Grade: An incomplete grade may be
assigned if the instructor concludes that a clearly identifiable
portion of course work can not be met by the student within the
academic term for unforeseen reasons. An incomplete WILL NOT
be given in the event students have not attended classes. NOTE:
only full time instructors are allowed to assign an incomplete
grade to a student. Please read form carefully for more information.
First Semester "WU" Grade
Deletion Petition: You may petition to have the first "U"
or "WU" grade(s) earned dropped from your GPA calculation. This
policy applies only to the first term in which
you received a
"U" or "WU" grade(s).
How to Apply for Reinstatement: These
forms are for students seeking reinstatement. Please print all
forms and read carefully. All required documents will need to
be attached before a CpE Advisor will consider reinstatement.
Readmission After Dismissal: These
forms are for students seeking readmission after dismissal. Please
print all forms and read carefully. All required documents will
need to be attached before a CpE Advisor will consider your request.
Academic Integrity Policy: CpE students are required
to adhere to University guidelines for academic integrity. Please
print and read carefully.
Graduation Petition: Please write legibly using black or
blue ink only. All information must be filled in completely
before the CpE Program Advisor can evaluate petition.
Update to Graduation
Petition - Substitutions and Waivers Form: Use this form for any changes to your approved Graduation
Petition - example: you decided to take a different elective
course that was not originally listed on your Grad. Petition.
