Read and follow all directions on the forms carefully. Always write legibly and use a black or blue ink pen. Incomplete forms will not be processed.

Add/
Drop Permit: Use this
form when you can not register through "My Sac State" and until the
University census date only. (click
here for census date)
Add/Drop Permit After Deadline: Use this form to drop courses only after the university census
date. NOTE: you will not be allowed to add courses after the
census date. (click
here for census date)
Continuous Enrollment Form: Use this form to maintain
your place at the university when you have completed all the
coursework but have not yet finished your thesis, or project.
Supervisory Course Petition: Use
this form to add supervised courses - CpE 199, 299 and 500.
Internship Packet : Use
these forms to add CpE 195, or CpE 295 - Fieldwork in CpE courses.
NOTE: You must have submitted your report and your supervisor's
evaluation for any previous internship period before being allowed
to register again for an internship.
MS in CpE Advising Form: Please fill in form completely
before seeing your CpE Academic Advisor.
Classification Application: For Conditionally
Classified and Unclassified graduate students. Once you have
completed your conditions, use this form to change your status
from Conditionally Classified or Unclassified to Fully Classified
status.
CpE WPE Waiver: Print
form and read carefully. Check the "Standards" section
to see if any of them apply to you, then follow directions on
form.
Substitution
or Waiver Petition: Use this form to establish
approval of course substitutions or waivers in your department
file. If this form is approved, you may list the substitution(s)
on your application for advancement to candidacy form.
Advancement to Candidacy: This
form allows you to enroll in 500-level units. Once approved,
the form represents a contract between you and the university
showing which courses you will need to complete in order
to earn your degree.
Petition for Exception: This form allows
you to make any course changes to your approved Advancement to Candidacy form.
MS
Graduation Application : Use this form to apply
for graduation. Please read carefully and follow directions on
the form, using a black or blue ink pen to fill in all information.
Repeat Grade Replacement Petition: For
a single course repeat, use this form. Please read form carefully,
and follow directions on form before turning it in to Admissions
and Records.
Petition for Incomplete Grade: An incomplete grade may be
assigned if the instructor concludes that a clearly identifiable
portion of course work can not be met by the student within the
academic term for unforeseen reasons. An incomplete WILL NOT
be given in the event students have not attended classes. NOTE:
only full time instructors are allowed to assign an incomplete
grade to a student. Please read form carefully for more information.
Academic Integrity Policy: CpE students are required
to adhere to University guidelines for academic integrity. Please
print and read carefully.
